Welcome every client suffering from various diseases. Please bring your insurance card.

You will be first asked to fill in the questioning sheet including your name,
birth place, birth date, past history of your illness, your complaint,
your history of allergy and so on.

Next you will be asked to measure your temperature and blood pressure.

Then please wait until your name is called.
